Phases of Consultative Services

CBI Consultative services include 5 distinct phases of involvement. These are: Assessment, Development and Implementation of the Support Plan, Monitoring, Follow Up, and Completion. CBI Consultants is a time limited service that proposes to complete a specific number of hours over a specific timeline. We are typically involved with families for a period of 1-3 years to complete all 5 phases. Our goal is to develop and support self contained teams. Our role would gradually shift from providing intensive supports to acting as a resource once the support plan has been established. The following describes the 5 phases of involvement:

Assessment • Develop personal profile for individual
• Complete all relevant assessment protocols and conduct a functional assessment
• Identify team members and clarify roles and responsibilities
• Identify potential barriers that may affect the development and implementation of the support plan
Development and Implementation of Support Plan • Support team to individualize strategies
• Train team to develop, implement, and adapt strategies
• Teach team how to problem solve
• Continue team building and support team members to work collaboratively
Monitoring of the Support Plan

• Continue to support team to modify or adapt strategies as needed
• Support team to develop self-monitoring tools
• CBI begins to act as a resource to the team and encourage independence
• CBI begins compiling a written training manual (with videotapes when applicable or necessary)

Follow Up Services • Continue to consult to individual team members or attend regular team meetings on an as needed basis
• Measure individual and family outcomes as identified in the assessment
• Provide short-term re-involvement for specific issues (i.e. transitions to high school or adult services)
Completion • Individual/Family receives a comprehensive training manual including all written reports, written strategies on all interventions strategies, team roles and responsibilities, monitoring and evaluative tools, and necessary checklist, and copies of all augmentative and alternative communication visual aides.
